Woodbridge Holistic Assessment Team (WHAT)
The WHAT project is around early intervention for first fallers, near misses, stumbles and people who are anxious around falling.
The aim is to have a one stop shop of professionals to undertake any assessments needed, offering advice, support and guidance to minimise the need for several appointments and visits to several different professionals, over several days with much longer waits, minimising the risk of further falls and offering a unique quality service for the people of Woodbridge.
Professionals and services include:
Pharmacist- to discuss medication and undertake a review, prescribing or deprescribing where needed, giving advice and guidance.
Nurse- to take blood pressure, weight and height, undertake a GP cog testing where appropriate and take bloods if necessary.
Therapist- Offering a balance assessment, exercises, signposting to community classes, arranging small pieces of equipment like walking aids, give advice and guidance around their home to encour/age a safer home environment.
Chiropodist- To check feet, undertake any treatment and give advice.
Social Prescriber/Community Connector- to offer support, further referrals. Support to book appointments such as eye tests and hearing tests. Any guidance around further opportunities in the community, classes, groups, social events. (Activities, SPOT Wellbeing, Suffolk MIND, One Life Suffolk (smoking cessation, weight management)).
Mental Health Support- Depression and anxiety screening
Feedback -
10 people gave feedback from the event. 3 of these people were carers/friends or family members for the customer. The majority of these questions were rated from 1 to 5. 5 being - Excellent/Highly likely and 1 being- no/confusing/inappropriate.
