Pneumonia Vaccine Survey

As an Integrated Care System, we are committed to ensuring eligible Chronic Obstructive Pulmonary Disease (COPD) patients have access to pneumonia vaccines and addressing heath inequalities, including improving the offer for people with a learning disability and autistic people so we have some questions to understand their experiences. 

This survey is to understand from your perspective the relevance of having the pneumonia vaccine if you have a diagnosis of COPD. We would like to understand what support you would need to make an informed decision and how you would like to access more information about the pneumonia vaccine.

 We would be interested to understand if you have experienced challenges or barriers in accessing the pneumonia vaccine service and what support is needed to address these issues.
