How we will make a difference

Actions:

  • To enable the ageing population to live a healthier life for longer, in the person's preferred place of residence, by anticipating the health, care and wellbeing needs of the population, identifying people at an earlier stage and providing a multi-disciplinary approach to their needs through the Neighbourhoods model from 2023.
  • People with frailty are supported at home and unnecessary hospital admissions are avoided by achieving the 70% two-hour target for urgent community response to deliver care in the patient's home from 2023.
  • Carers feel supported by the system and empowered to continue in their caring role by increasing the number of carers identified with their GP practice and signposting to local organisations to deliver support from 2023.
  • To have an active ageing population by improving the amount of referrals into the Strength and Balance programme and boosting delivery to more residents, including care homes, from 2023.

  • Increased disability-free life expectancy at age 65 by 2028.
  • Reduced rates of severe frailty or increasing frailty by 2028.
  • Fewer emergency hospital admissions due to falls in people aged 65 and over by 2028.
  • More carers identified and supported by 2028.

  • Higher rates of people with joined up mental and physical health services for the older population by 2028.
