JFP: Age Well - Carers

Consultation has concluded

How we will make a difference

Actions:

  • Carers are identified at the earliest opportunity.
  • Carers can easily access information, advice and support when needed. Young carers will have the same priority as adult carers and are supported through appropriate transition resources to prepare them for adult caring responsibilities.
  • We have systems and services in place, which are developed and influenced by our local carers, that work for, and support, carers.
  • Carers can enjoy improved health and well-being across Suffolk and north east Essex.

How we can work together to know we have made a difference

The difference that you will see ...

  • Carers can report that they feel recognised and supported via established surveys and provider feedback.
  • You are seeing improved signposting to carers’ assessments with tailored support packages and ongoing review as required.
  • Carers’ report improved access to information.
