  • Children, adults and older people are supported to make healthy food and drink choices.
  • Children, adults and older people are supported to be physically active.
  • Children, adults and older people are supported to get the sleep that they need.
  • Young people, adults and older people have good sexual and reproductive health.
  • Children, adults and older people feel safe in their home and community.
  • Children, adults and older people are socially connected and avoid loneliness.
  • Children, adults and older people have opportunities to volunteer to help them connect with, and support others.
  • Children, adults and older people have opportunities to connect, learn and take notice.
  • Children, adults and older people are able to live in a clean and sustainable environment.


How we can work together to know we have made a difference

The difference that we will measure ...


  • Healthy life expectancy for males and females will increase and the social gradient in healthy life expectancy will reduce over the course of this strategy to 2029.
  • The proportion of adults (aged 18+) classified as overweight or obese will reduce.
  • The prevalence of overweight and obesity in children in reception and year 6 of primary school will reduce.
  • The rate of increase in the incidence of type 2 diabetes will reduce.
  • The proportion of the population (adults and children) meeting the recommended ‘5-a-day’ on a usual day will have increased.
  • The percentage of children and adults meeting the recommended levels of physical activity will have increased.
  • Rates of emergency admissions due to falls will reduce.
  • Total prescribed Long Acting Reversible Contraception (LARC) will increase.
  • Fewer people requiring abortion services.
  • Fewer conceptions in those aged under 18 years.



The difference that you will see ...

  • More people will report better quality of sleep.
  • More children and adults walking or cycling for travel at least three days per week.
  • Greater utilisation of outdoor space for exercise/ health reasons.
  • Fewer people who report low satisfaction, worthwhile or happiness, or higher anxiety.
  • Percentage of adults who report feeling lonely often or always will reduce.
  • Rates of self-reported wellbeing will improve.
  • More adult social care users and adult carers who have as much social contact as they would like.
  • More people aged 40-74 offered and taking up an NHS Health Check.
