  • An increase in active personalised care and support plans.
  • An increase in the number of personal health budgets (PHBs) in place.
  • An increase in the number of referrals into social prescribing link workers.
  • An increase in the number of people whose case has been discussed as part of a shared decision-making process in primary care.
  • By 2028, we expect that personalisation and personalised approaches are embedded across our whole system and becomes the default way of working.

  • A broader range of opportunities to take a PHB on offer.
  • An increase in the number of people who have had a consultation with a Health and Wellbeing Coach or Care Co-ordinator.
