Did you know that your data including your age, gender and prescribed medications is used by your local NHS to plan its services?
Your data is also used by your GP practice to help identify patients at higher risk of certain medical conditions, prevent unnecessary hospital visits and support people with long-term health conditions.
This collection and use of your data is called risk stratification.
We have published a leaflet called “Using Your Data To Plan And Support Better Care” which we encourage you to read. It explains how risk stratification is an important tool for the NHS, helping deliver the best services.
The leaflet also explains how you have a right to opt out if you do not want your data used in this way and how that decision could affect your care. You can download the leaflet by clicking on the Documents link to the right of this page.
