How we will make a difference

Actions:

  • Provide primary and/or community-based services to keep people healthy in the community.
  • High quality services provided to adults with learning disability and/or autism.
  • Provide good quality care and health services to work in an integrated way to optimise outcomes.
  • Action will be taken to tackle the causes of morbidity and preventable deaths in people with a learning disability and/or autism.
  • The whole NHS will improve its understanding of the needs of people with learning disabilities and/or autism and work together to improve their health and wellbeing.
  • Increased investment in intensive, crisis and forensic community support.
  • We will focus on improving the quality of inpatient care across the NHS and independent sector.



How we can work together to know we have made a difference

The difference that we will measure ...


  • Annual Health Checks: a minimum of 75% of people aged 14 and over on GP learning disability register will have had an annual health check by 2023-24.
  • Learning from Lives and Deaths - Learning Disability Mortality Reviews (LeDeR): A continued focus on achieving LeDeR reviews to ensure that 100% in-scope reviews are completed in 6 months.
  • Inpatients: For everyone million adults, there will be no more than 30 people with a learning disability and/or autism cared for in an inpatient unit by 2023-24.
  • All Care (Education) and Treatment Reviews (C(E)TRS) Standards are met NHS England » Care, Education and Treatment Reviews (CETRs)


The difference that you will see ...

  • Autism: For people who have been referred to an autism diagnosis service, SNEE ICB will ensure that people wait no longer than 18 weeks from referral to first appointment by 2024-25.
