The current Suffolk (excluding Waveney) Community Service concludes at the end of September 2027.


The future community service will not promise to solve all problems, or cure all illnesses, but will be a step towards shaping the future to help people stay well for longer.

The current provision is worth approximately £100m per year and is finishing its 10-year period.

There are several important national changes that will have an impact on this over the next 2 years, including:

  • NHS 10 Year Health Plan for England: Fit for the Future
  • Local Government Reform


What is in our current community services?

Community Services are a broad range of healthcare services delivered outside of hospitals, supporting people across all life stages—from newborns to those at the end of life. These services help individuals stay independent and healthy at home, manage long-term conditions, and access specialist care. They are often delivered by multidisciplinary teams and include both planned and urgent care, rehabilitation, palliative care, and support for complex needs. Current Approach:

  • All ages - cradle to grave
  • Complex care at home
  • Care closer to home
  • Health promotion

Reflections on current contract arrangements:

  • Historic contract with multiple contract variations in place
  • Not reflective of changes in healthcare delivery
  • Funding is currently from multiple sources
  • Unclear naming conventions and terminology

Current providers:

  • ESNEFT - East Suffolk and North Essex Foundation Trust
  • WSFT - West Suffolk Foundation Trust
  • NSFT - Norfolk and Suffolk Foundation Trust
  • Suffolk GP Federation
  • Suffolk County Council

We would like you and your communities’ help in designing a public engagement and co-production on the key themes, principles and approach to the future Suffolk Community Services model.

We need to consider how we get feedback and input from people of all ages across Suffolk. We especially need to ensure we include children, teens, parents, guardians, carers and wider family networks, and those who might suffer the highest impact of health inequalities.

As community services cover such a large number of organisations, teams and different clinical pathways, we will also be able to use health data and analysis to help us know if services are right for local people.

  • We would like to encourage a more proactive, preventative, family/carer orientated approach to care for all ages. Supporting self-management, early identification and joined up care when needed. 

    Will the general public understand what is currently known as community health services?

    Should we use the term neighbourhood rather than community health service?


    We are proposing a short but focussed list of questions, some early suggestions include:

    What should we do more of in the future community health service?

    What should the key principles of working with people be?

    What's important to people? What's important to me?

  • The I&O Group would have representation from: 

    • Commissioners and those responsible for current Suffolk’s Community Services 
    • Current service providers providing links to current service users and workforce 
    • Community-based groups, people and organisations representing those impacted by health inequalities related to Community Services, for example experiences related to: 

    • Children, young people and transition 

    • Family carers 

    • Mental health 

    • Neurological health 

    • Learning disability 

    • Frailty and dementia 

    • Neurodivergence 

    • Alcohol/substance use  

    • Homelessness  

    • Living in a rural area and/or no access to affordable transport
